Please contact the caller to introduce yourself and confirm the assessment date/time. **Please confirm the CSM completing the assessment** *Assessment Date and Time: *Friday, 9/26, 12PM *Client Name: *Carla Cox *Chatter Notes:* Kiana transferred Jennifer to me to set up the assessment. Jennifer let me know that she lives out of state and is visiting her mom the week of 9/22 and would prefer to do the assessment at the end of the week. She requested this for Friday, 9/26 at 12PM. Her mom's current CG Dawn will be there at this time as well, which she feels like with help when going over the assessment details. Carla currently has 2 caregivers that they love, but they were not able to do the additional weekend coverage that they are needing, so they started looking into agencies. Jennifer mentioned that they used an agency before and had a very bad experience with which caregivers were being matched up. I went over our screening process and how we match up both on skill set and personality through the customized care plan. Jennifer said she is still very weary about this and wanted to know if they could interview the caregivers beforehand. I let her know that during the assessment we could discuss the possibility of a meet and greet shift where care is provided for a shorter period of time the first time to provide them with some assurance it's a good fit. I let her know there still would be a charge for this, which she understood, and that it would need to be confirmed with the CSM. Jennifer wanted to make sure we were aware of her mom's anxiety, depression, and OCD that she had discussed with Kiana and that one of the most important things is having a CG who understands this and be there for support/companionship for her mom. She would like the caregiver to provide transportation for outings and errands using Carla's car. Jennifer said that Carla does like to go to church and asked if this is somewhere the caregiver would still be able to assist. I let her know that we couldn't match up caregivers based on religious affiliation, but we can provide transportation and accompany her mom for care to any outings/activities including this.
