Re: Uber account

Whoops, that went out before I was done typing!  Yikes!  Sorry about the
all caps, was going to re-write that and somehow managed to push the send
button instead.  The previously  mentioned form is now attached, and it is
important to use the exact list on the Locations tab as the program needs
to match the full location (it cannot figure out shorthand or general
references).  If you would like any individual to have access to more than
one location that isn't an issue, just list the locations off to the right
in the same row for that person.  Please put one location per cell.
