Delayed due to family managing her husband who is ill. Children needing time with him will then revisit long term care for their mother. Geraldine Kiser had a medical procedure yesterday. She had requested we contact her after this procedure so that she is able to make a more informed decision regarding her care needs. Jinny is checking in with her today, Wednesday, 11/5. Marilyn and Chuck Rivkin viable opportunity but both parities are resistant to care for themselves. They see the need for the other but will not accept it for themselves. Chuck currently requires more care than Marilyn. Venus is revisiting with the couple's children today, Wednesday, 11/5 Dr. Shulman - Mrs Shulman stopped responding to Malin's outreach. Malin is reaching out to Bonnie, the Wellness Nurse, at The Mather today and will provide an update after that conversation. Charlene Eberstein - closed, post-assessment. Charlene's daughter did not show for the assessment. Malin and Charlene waited for approximately 45 minutes before Charlene could contact her daughter. Charlene's daughter informed her at that time that she would take a leave of absence from her employment to care for Charlene. Andrea Heard - Assessment pushed back from November 3 to November 6 Mary Lou Doherty - Phone assessment completed on 11/3 with opportunity for a fast start. Contact was also speaking to two other agencies. We sent our CCA for review. Malin will follow up today, Wednesday, 11/5. Ted Schultz - Phone assessment completed on Tuesday, 11/4. The client won't be discharged home before 11/15. Venus will pop in to introduce herself while Opportunity convalesces at The Clare rehab.
