Insurance Renewal Updates Forms for Home Care Assistance Toronto

The group insurance policy for HCA Home Care Assistance is coming up for renewal on April 30th and to date we have not received any information from the HCA Home Care Assistance Canada contacts we have on file. Are you able to assist with the 2022 Renewal Information for Home Care Assistance Toronto o/b SJD Care Services Inc. and Mavencare Inc.?

Please find attached the forms detailing the information required in order to prepare for your upcoming renewal.

Attached are:

  1.  Renewal Updates Form - please review, make any corrections as required, complete the updates information, sign and return the form.


  1.  Statement of Values Form - your policy currently carries a Co-Insurance Waiver. Please complete the form by reviewing the limits for the Toronto location only (ignore the other locations), revise as required, sign and return BOTH pages along with the updates form.

NOTE: due to the unusual corporate structure for HCA Home Care Assistance Canada Inc. We WILL NOT BE ABLE TO PROVIDE A GROUP POLICY FOR 2022/23. INSTEAD EACH BRANCH WILL NEED TO CARRY THEIR OWN POLICY AND HCA HOME CARE ASSISTANCE CANADA WILL BE SHOWN AS A NAMED INSURED ON THE POLICY.
