Circling back to this, I believe Tim is looking to ensure the form is approved by counsel before we start distribution to the field/asking clients to sign. Are you able to review this?

Hi Steve,

 

Sorry to bother you again. I would like to create a waiver like this for all Canadian locations. In short, if a client received a government funded home care service (at their private dwelling) any additional care is considered exempt from sales tax. Should the service ever be cancelled, it would be the responsibility of the client to inform us of that change so we can reinstate the sales tax.

 

Can you assist with this?
