Telephone Answering Service Forms

Thank you for choosing Professional Communications Messaging Service, for
your telephone answering service needs!  Attached are the information forms,
and billing information form I will need for you to fill out and fax or
email back to me.  When filling out the forms, feel free to attach
additional sheets, flow charts, or any other information that you need us to
have to build the account.  Once I have the completed forms, I will forward
the contract to you.  Our contract runs on a monthly basis, with a thirty
(30) day written notice of cancellation.  To start the service, our
corporate office will need the information forms, billing info form, the
signed contract, and a check, either faxed or mailed to them.   The address
is listed below.  You may also charge the total amount to your credit card.
I will need to know which base rate, and options you wish to utilize.  Keep
in mind that we will review this every month, and if need be will place the
account at the most cost effective package!

 

To start service we require an initial payment which includes your first
month payment, a minimum $50.00 activation/set-up fee, and the applicable
taxes.   If you are not enrolling in automatic payments to your credit card,
or via ACH, we also require one month's payment(equal to your first month),
which is held as a deposit.
