Hi Tim,

In order to save time for the Care Managers and be more efficient in contacting the clients, the CM's only need to submit this form as soon as they have a lead.

In an effort to simplify the process as much as possible for both the case managers and the clients, we have created a lead form that will allow you to enter minimal information that will then automatically be transferred to the Lifeguard Team.
