Fwd: CCA REVISIONS - Montreal suggested edits

Hi Tim,

Steve and I met yesterday and went over the Montreal CCA one more time. We
changed the limitation of liability back to the original language and
updated the mileage reimbursement language with the Canada Revenue Agency
so the amount to be reimbursed will always be up to date. Here's the link

Will you please look over this CCA once more and give us your feedback?
Once you confirm it's good to go, will you please have this translated into
French? I'll start changing the other Canada entity agreements to conform
to the Montreal template. I'll let you know if there are differences in the
other documents that need to be reviewed.

Tim,
One additional Payment Authorization form for your review if you are adding
a fee. Vancouver's Payment Authorization form has both ACH and Credit Card
options listed.
