Yes, Denise has agreed.
She can fill out the form as soon as you have it available. We would just
need to know when the date the service would start, the amount per hour,
amount of hours per week and how long it would take for her to receive her
first payment. We ask that because sometimes it can take as long as 4 weeks
at times.
