Hi Cheryl,

Brian and I did this consultation on January 10th. The family wanted to
start ASAP at 40 hours per week. We could not even start this case.

I know you want to be appraised of situations like these--very unfortunate.

Hi Denis and Brian I thought I would send you a quick email since I have
been tied up in meetings all day today and will be tied up tonight. I
didn’t want you to go any further I have found a care worker which was a
referral from my sister in law who knows her well and it just came up this
weekend because her client just passed away. I would like to still keep my
contact open with your agency in case she doesn’t work out if that is
possible. I did feel comfortable with your agency but this one particular
home care worker was he we originally we wanting but wasn’t available at
the time and we weren’t able to wait for her.
Thank you for all your time you have spent on my file and we may still be
in touch with you.

I am more than happy to answer your questions:

1. Monthly cost is calculated as the difference between our $35/hour rate
and the $21.40/hour funded by the WRHA x 40 hours per week x 4.3 (the
average number of weeks in a month)

($35-$21.40) x 40 x 4.3 = $2339.20 (average monthly cost)

2. In terms of timing of the WRHA payment and our invoicing, we will wait
until confirmation that the account is funded before withdrawing money.
Most of our clients receive funding from SFMC and we work with them to
ensure things run smoothly with respect to invoicing and payment.

3. The "caregiver agreement" clause is in the consent agreement because we
spend a huge amount of time and resources recruiting, training and
nurturing our caregivers. If families wish to hire these employees
privately before the 3 year period is over, there is a $10,000 buyout fee.
FYI, this has never happened before.

4. Your family can terminate services at any time with at least 24 hours'
notice without any penalty. While we hope to establish a long-term
relationship, we do not feel it is fair to lock families into a contract.

Thank you for your questions. Hopefully I have answered theme to your
satisfaction, but please let me know if there is anything else. Please get
the consent agreements back to us as we have an amazing caregiver in mind
and are looking to start care very soon. Brian will be in touch with the
