Action Plan
Continue the weekly/bi-weekly CGM meeting with the office on Tuesday for a
Lunch and Learn.
Continue Weekly deep dive:
Clients needing more hours
Two clients will increase to 24/7 care while their spouses are traveling.
One client will add 25 hours a week. But, her tried and true caregiver has
resigned so she is waiting to ensure we have a match.
Winback from closed lost- calling two who chose private acregivers to
ensure they are happy
Projected revenue for this week is up slightly to $266,000
Twenty-seven assessments completed for this month
10 SOCs for the month, two more starting next week
8 assessments were conducted last week; five have started and one starts
next week
9 assessments are scheduled for this week, 7 are complete, and two are
scheduled for today. 1 starting next week
Four SOCs this week
The monthly SOC goal is 17, we're still hopeful we will hit the target.
Twelve assessments complete. Opportunity is still ruminating.
1. Four: resistant to care, slow to nurture.
2. Five:will start in December. One will be couples care and a tier 1
3. Three: family is discussing direction they want to go
Controllable Deactivation
1. One: Client is very happy but the daughter wanted someone for her
benefit—someone to do family laundry, cook for the family, and clean for
the family. She wasn't satisfied with caregiver spending time and attention
only on the client. (Donat)
2. One: Client deactivated due to 90 days of no service activity—an
intermittent client who calls for rides (Galbraith).
3. One: client never started but still may—they have been active for 90
days. The family asked us to keep in touch because they foresee a future
need. -( Meyer)
4. One: Client was short term, Tier 3 client whose condition improved
and wife is now able to manage care.(Mucha)

Schultz - Releasing from rehab - wife is appealing to try and keep him in
rehab
