Good Day!      Following an audit of the payment authorization form for client Sylvia Priebe a financial Power of Attorney (POA) is required. If you're are unable to obtain the POA from the signer, a new payment authorization will be needed from the individual who provided their bank information.     Please Note: Before uploading into ClearCare, please review the CCA and payment authorization to ensure the onboarding forms meet compliance standards.
