With tax season in full swing, TheKey Benefits team is requesting all team
members and Caregivers be aware of Form 1095-C, a document provided by the
Internal Revenue Service, distributed electronically by the close of
business today. Form 1095-C is required by law to be sent to employees and
provides information about health coverage offered by TheKey.
If you declined to sign up for a health plan with TheKey, you will still
receive Form 1095-C as documentation of coverage offered.
Please Note: You do not need to submit copies of your Form 1095-C when you
file your federal taxes; they are for your information and records only.You
can learn more about Form 1095-C on the IRS website

Form 1095-C will indicate your name, the name of your employer, the months
during the prior calendar year when you were eligible for coverage, and the
cost of the lowest monthly premium you could have paid for coverage under
TheKey’s health plan options.
We encourage you to keep this form with your tax records.
To access your 1095-C Form:
1. Login to Paychex Flex

2. On the Dashboard, there is a “Tax Documents” tile which includes the
1095-C
3. Click the “2022 1095-C” PDF icon to download your 2022 1095-C document
Please disregard any W-2s from TheKey Management LLC that may also be in
the “Tax Documents” section of Paychex Flex. These documents are not valid
since the W-2s for TheKey Management LLC were created and distributed from
Workday. We did not have the option of excluding these particular W-2
documents.
