Hi Tim,

I have attached to this e-mail a zip folder containing documents for your appointment with Teio next week together with documents that will require your original wet signature.

First, the 6 statutory declarations, are the documents that you will remotely swear in the presence of Teio on Tuesday December 23.  Please review the documents and if they are acceptable, would you please have each document printed for your appointment on Tuesday.

The package also includes the following:


  1.  Articles of Continuance, would you kindly sign 6 times in the table on page 2 (once for each company that is amalgamating).
  2.  Application for Supplementary Certificate, would you kindly sign at the bottom of page 2.
  3.  Initial Registered Office and Notice of First Directors – would you kindly sign at the bottom of page 1.

As the registry requires wet-ink signatures, would you kindly sign each of the above forms and send a scan of the signed forms to my attention with the originals to follow by mail.
