GM Sir,

I’m sure Cheryl will raise this to you, but wanted to be proactive by
sharing. I’ll push my team to lean in here and help, but given this is
somewhat new territory I’m sure we’ll need Cheryl to help us through it.

More to come.

If you could please call Blue Cross and ask them to send you an
authorization - this should include the policy number, number of hours and
forms that need to be completed. I do know things are done differently in
Canada and I wish I could help you out more, we will learn together how to
handle this claim.

Chuck's clients are completely different. Those clients are part of a
government program. We do not have that here in Ontario.

What form do you need filled out each month? We do not receive the same
form as Alberta because again, this is private insurance not part of a
program.

The client is not a veteran so there is no VA involved in this case.

I will sign this form and send it to Blue Cross but how does that change
how TheKey bills? I need to know what you need in order to submit the bills
to the insurance company.

Hey Cheryl, we will do whatever we can do to help you, however, we will
need the form that needs to be completed each month, plus the information I
have requested in this stream.. With Chuck's clients, he provides us with
the billing information and forms, we complete at the end of the month,
download it to his files and Chuck prints and signs the form, mails it for
us. Since you are the General Manager, you can sign the form given. We do
want to set this up properly and with your help, we can achieve this. Let
me know if you have any questions, or how we can help on this end. Your
Veterans billing is quite different from ours, so we need to make sure we
get this right the first time around.

Kristy, please put a hold on this client until we can get the information
needed to properly bill. Thank you Cheryl for any help you can provide.

Hi Edie,

No we can't continue to bill the client. We are going to lose this client
if we do that. It took a long time to set up direct billing and we need to
bill Blue Cross. Please, this is a really good client and he has been very
patient with us.

I should not be filling out the form. I am not involved in billing in any
capacity. The form should be signed by someone who oversees this area of
the business.

I have already called Blue Cross and added him to our client list. They are
ready to be billed directly, it was TheKey who asked for a letter from Blue
Cross.

What do we need to do in order for TheKey to submit bills directly to Blue
Cross. What do you need and I will do it but please do not bill the client
anymore.

Good morning Cheryl. Thank you for the information. You can go ahead and
sign the form. We will continue to bill the client. If any changes need to
be made, please let us know. Have a great day!

Hi Edie,

We don't have any information on Vetarans status. The client has never
shared an ID with us.

I have no idea what the authorized hours are as those were not shared with
us. The client has always oaid us and would then submit the bill to Blue
Cross.

There was a form that was part of a previous thread that needed to be
signed by TheKey and the client. The client had already signed it. I have
attached it again here.

His ID and Member number were also in the other thread but here it is
again. Policy number is 00915 28100. His identification number is
00040162501

Hey there Cheryl, we are preparing the above mentioned client for billing
and want to make sure we do this right the first time. I need the
following information:

   1. Is the Veterans ID 00060162501 - I want to make sure this is the
correct number for Veterans filing - or is there a different number that
   starts with K
   2. What are the authorized hours (per day, per month or per year)
   3. Do you have a submission form that needs to be completed with service
hours provided? Is this to be completed Weekly or Monthly. If so, can you
   please download into file.
   4. Would you like us to complete the form, download to file and have you
   sign and mail the forms?
   5. Would you please provide us with the provider number

I would appreciate it very much if you could provide this information ASAP,
so we can set this client up properly and get the billing done. Thank you
for all your help.
