Quebec

Please find, for your review and execution on page 1, where highlighted, a declaration to update the registration in Quebec.  If any of the information is incorrect, please let us know and we will arrange to have the form updated.

We do not require the return of the original of the Quebec form, you could either sign and return a scanned copy of same to my attention or I can send the form to you through Docusign.

Manitoba

To update the registration in Manitoba, please find attached the following:


  1.  Application for Supplementary Certificate - Continuation;
  2.  Application for Supplementary Certificate - Amalgamation; and
  3.  Power of Attorney.

The Registry in Manotiba requires the return of originals, would you kindly sign and date the attached and forward the originals to my attention.

There is a separate package of documents that we will send through for signature through Docusign either later today or tomorrow.
