Here is the info to get your contractor set up with an email address and
then access to other systems.



Best practice dictates that I will need the following information for each
contractor:

   - *Signed statement of work and fees document* (one per standalone
   contractor or one for a MSA with multiple contractors; if your consultants
   do not have a signed agreement template of their own, they can utilize the
   consultant agreement template attached)
   - *Signed Independent Contractor NDA* (one per individual-required)
   - *Signed W-9 form*
   - *Data from the Grid Below* (per individual)
