Given his history of behavioral outbursts, physical restraints, and cognitive instability, providing clinical oversight will be essential to guide the caregivers and ensure they have the necessary direction to manage his de-escalation needs. My goal is to provide the team with consistent support to prevent caregiver turnover and ensure a successful transition as he moves into skilled nursing. Please let me know how I can best coordinate with you and Rose on this. Keep me updated with assessment date and time.
