Here is the email with attachments. The email I sent to Steve did not include the actual policy document – just the invoice and certificates and it would seem that I missed a letter in your email address (which explains why you didn’t receive it)

I have the renewal updates forms for all the offices to send – should these also come to you for distribution?

  *   Mavencare operations have been added to the HCA Home Care Assistance (Toronto) group policy effective March 14, 2022
  *   A revised Proof of Insurance is included for your use
  *   NOTE: the coverage under this policy expires on April 30th, 2022 so the premium established for this change has been pro-rated for 6 weeks (March 14th to April 30th)
  *   We ask that you please review the attached document carefully and advise us immediately if any changes/corrections are required. It is important that you keep this document safe and maintain it for future reference.
