RE: LHC & Orofresh's EHTs

Hi everyone,

May I suggest we contact Ali for further explanation, as the EHT should technically have been merged with Orofresh, thus the contact information on that account should be the one that moves forward.  I would be happy to call him and ask for details on what he is requesting.

In the meanwhile, I wanted to clarify that the person who signs the form (part 4) authorizing the representatives has to be a director.

For Part 2 of the from, I think Humanity Financial (myself or Megan) should be added, plus Hua and Carolina.  This will allow all/any of us to be able to have access to the EHT account.

Please let me know if you would like me to contact Ali and to complete the form for signatures?

It’s my understanding that LOVING HOME CARES SERVICES LTD amalgamated with OROFRESH ENTERPRISES INC on Jan 1, 2022, and as result, the authorized contact person on the EHT account is no longer a contact at your company; thus, we will need a new contact when the ministry needs to contact you with regards to the account.  Please kindly submit a completed and signed FIN 146 (attached).

On Part 2, under “account authorizations” please enter “Employer Health Tax Act” and below that, enter “All”.

On Part 4, the person who signs the form must enter their full name under “Full legal name of taxpayer”. Please note that the form will need to be signed by owner or director with signing authority verifiable through the BC Registry.
