Up until June 1st, I believed that the bonus plan was structured the way it was presented in our signed PandaDoc agreement (Email 2). Based on the DART team's response (Email 3), it seems like the goalposts have been moved again making it harder and harder for the Care Solutions Team to 1. Be able to calculate their bonuses and 2. Make a fair bonus given the structure in the past. If possible, I would like to set up a meeting to discuss and get clarification for myself and my team on this bonus structure and the new bonus structure (as of June 1st). Thank you, ------------------------------------------------------------------- *Email 1: (From Joey to Dart Team)* "Hi, Sarah, My team had the following questions/issues with the May Proofs: *Dennis Fancy* I found three "Bees" that were "Cheetahs" according to the model before June 1st. 1. Blessing (May) 2. Hanna (April) 3. Robinson (April) ------------------------------------------------------------------------ *Taylor Cleland* 1. It looks like Anthony Griese is still not populating as a start of care in AP. 2. Client Aziz was 24 hours of care post surgery so based on the plan in April/ May that should be classified as a cheetah but it's showing it as a bee. 3. Helen K- we had 45.5 hours of care over 5 weeks then they put care on hold for 3 weeks but now have an ongoing schedule in place. Does this still count as a bee (In the worst case it should be a cheetah) -------------------------------------------------------------------------- *Desiree Aranjo:* 1. Annette Charest Savoie - they had total 24 hours of care - *Cheetah* - noted as bee 2. Brenda Lamb - they had 36 hours of care - *Cheetah* - noted as bee 3. Janis O'Neill - they had 28 hours of care - *Cheetah* - noted as bee 4. Joseph & Francoise Thibeault - they had 24 hours of care - *Cheetah* - noted as bee 5. Gilles Gregoire* - *Cheetah* - noted as bee --> This was an approved Cheetah based on an email from Angie Markwell" *Email 2 (Screenshot of bonus plan month of May):* [image: image.png] *Email 3 (Response from Dart Team):* "Hi Joey, The DART team calculates utilization within the first 14 days (two weeks). They combine 20 hours (week 1) + 20 hours (week 2) to get to the 40 hours. The idea behind this is that one week might have more hours than the other. Being over or under the 40 mark determines Cheetah instead of Bee or Elephant instead of Turtle. They then also look at whether they are clients that are over/under the 31 day mark to determine Elephant instead of Cheetah or Turtle instead of bee. That's why they actualize 49 days after the SOC to give in a buffer to determine tenure. Starting with June bonuses we will be calculating as such: The DAR
