Hi Timothy,      Can you provide a completed copy of the Vendor’s W9? Enclosed please find IRS Form W-9, “Request for Taxpayer Identification Number and Certification”.   We are required by the Internal Revenue Service to obtain this
information from the vendor to determine if we must issue a 1099 at the
