Please contact the caller to introduce yourself and confirm the assessment date/time. **Please confirm the CSM completing assessment. ** Assessment Date and Time: August 7, 1PM Client Name: Allie Colvett Chatter Notes: GenWorth LTC. Sue has not spoken with Care Scout yet. She may appreciate guidance in initiating the Long Term Care policy. Sue reached out to TheKey for her mother, Allie 100 years old. Allie is in good health, takes no medications, does not see the doctor. She lives in her own home and Sue is her primary caregiver. Sue describes Allie as being pleasantly confused and needs assistance and verbal queuing to complete most ADLS. Sue helps her mother with her meals, showers, and errands. Allie is mobile with minimal assistance. When out of the home she uses a cane and someone holding an arm , otherwise she uses a walker in her home . She is hard of hearing but has hearing aids.S he will not be able to use them without assistance. Allie is very sweet and communicates well but Sue feels she is lonely. We discussed the importance of regular engagement and its relation to healthy aging. I told her I thought it would be advantageous to have a second person for Allie to spend time with. I explained our personalized care plans that will include her mother's past interests, hobbies, favorite songs and books, in order for us to make a deeper connection with her. Sue also stated that she likes to travel and that she'd like to be able to call on an agency at those times that she wants to take a week vacation or weekend vacations. I explained to Sue that TheKey is a long term care solution for families. We require a set schedule. I reviewed minimum hours (12per week) and rates $36-$40. However, once Allie is a client, Sue is welcome to call on TheKey to add hours in the event she will be out of town, or if Sue is not feeling well and cannot tend to her mother, or she simply needs a break. Sue thought that sounded like an excellent plan.
