Chatter Notes: Client has dementia. Had a brain surgery, removal of
mengioma (fatty tissue), and has gone downhill since. Client is bed bound
and is on hospice. Primary cg is husband. Had a bed sore that "went bad"
and is above her coccyx. Caused a sinus in her back which has new skin
tissue growth and is not open to infections. May need a change of dressing
to that area if client had a bowel movement that gets into the wound, but
doesn't happen often - just changing a bandage. Has hoya lift but husband
only uses it - cg won't need to use it. Has a Jerry chair that husband also
works. Client has a cg now but he said the management is terrible and the
caregivers are not trained.
