[image: image] Setting Relevant Care Goals – TheKey Home Care Essentials Series Episode 1 Hello Team, Welcome to Week 1 of TheKey Home Care Essentials! We’re thrilled to launch this series with a powerful topic central to our client care: Setting Relevant Care Goals. This first session focuses on engaging clients and families in meaningful goal-setting at the assessment stage to drive better care outcomes, differentiate our approach, and align expectations. This Week's Content: This week’s material is a 15-minute audio recording on Goal Setting at the Time of Assessment. It’s designed to be concise and impactful—perfect for listening on the go or during a break. Access the Content Here <https://youtu.be/dYFHIFsVFn8> Discussion Questions for Your Pipeline Meeting: As you listen, consider these questions to discuss in your pipeline meeting: - How do you ensure family members are aligned with care goals, especially when their priorities differ from the client’s? - How can setting short-term goals help build momentum toward long-term success for the client? Your insights will help make these sessions meaningful and actionable. Weekly Google Form for Team Engagement: After your discussion, please take a moment to complete the engagement form. Each week, the GM is responsible for ensuring completion, though they may designate a team member to fill it out. This week there are 2 brief engagement questions: 1. For new /upcoming assessments: Identify opportunities to focus on goal setting as a way to demonstrate value and differentiate TheKey. 2. For Existing Clients: Go back to any Tier 1 or Tier 2 clients who you think may benefit from updated goals for a QA visit or reassessment to set new goals. Please use the space below to summarize activity and list any wins. This series is about meaningful learning and practical application, so let’s stay engaged and work together to elevate our standard of care. Thank you for being a part of this journey. We’re looking forward to your thoughts and insights in our first discussion!
