As the Community Associates at 401 N. Michigan, we will be taking care of
all your business needs from now on.

Details of your virtual office set-up are below. If you want to change any
of these details, add services, or need more information on parking and
administrative support, just call or email us directly.

Attached is the *1583 Form* we need to start collecting your mail. If we do
not receive this form, we *CANNOT* collect any mail. Please let us know if
you have any questions on how to fill it out.

We do require auto-pay to be set up in your myregus account. *Failure* to set
up your auto-pay in a timely manner will result in an invoice on the
account reflecting a one-month retainer.

To log into your account and pay your first invoice, please go to
www.myregus.com enter your email address and create a password.

Once payment is received, please see the attached USPS 1583 Form that we
will need to start collecting your mail. The address you will use is *401 N.
Michigan Ave Chicago, IL 60611 Suite 1200* (Please do not forget the suite
number since there are other floors in this building)

Mail handling:

   - Your company address corresponds to the one on your service agreement.

   - For your convenience, *we have automatically set up weekly mail
   forwarding at a price of $30 per month*. This all-inclusive package
   covers postage, envelopes and administration. Please send me the address
   you would like us to use or if you would like to change service to one of
   the following :
   - Weekly mail forwarding - $35.00/month ( automatically added)
         - Monthly mail forwarding - $ 19.00/month
         - Collect from center - $9.00/month
         - Opting out from the above services for mail will result in any
         mail received being forwarded to you the same day it was
received with a
         fee of $19.00.

What we need from you;

   - *Pay the first invoice (we do charge a month in advance, so there
   might be 2 invoices on the account)*
   - * Set up the account for autopayment*
   - *Complete the 1583 Form*
   - *2 Forms of ID*
   - *Mail forwarding address*

The center hours are Monday thru Friday 8:30 to 5:00. We are usually off on
Holidays and we will send a reminder email the day before.
