I have been looking through my drive and it doesn't seem to be shared with me and I need one done tomorrow (It was mentioned on my coaching call today that I should jump in to at least start the conversation and get information so that is my goal here to keep things moving forward). This lead is Diana Gray at Chartwell Harbours. Her needs are: - Housekeeping - Groceries (just get them for her, not going with the caregiver) - Optometrist appointments - Shower support - Had a major stroke and a series of mini strokes since - constant pain now = grumpy sometimes, cheerful other time. - Currently has Chartwell Harbours deliver ALL her meals - socially isolated - High fall risk (and knows it, she is very cognizant of her current conditions) - Son has mental health concerns, granddaughter takes the lead (Colleen) - Emotional distress about AHS services - changing caregivers - Finances are NOT an issue - all about caregiver matching - She prefers late mornings. I know I can get there by tomorrow for the initial consultation, I just need the paperwork if you've got it for me and you can fill in the blanks for supporting the start of care.
