You can download this from the Intranet > Interventionist Program (folder) > CTM Tri-fold Brochure Interventionist. You can also view the "FAQ_CTM" sheet, which also has a small section about the target audience. Unfortunately, there isn't a more specific document. But, I do agree that CTM is a great tool to use with someone who is experiencing social isolation (whether or not they have a medical diagnosis). The program offers several social and recreational activities (almost endless). You can be very creative with the intervention plan- include a variety of social activities or field trips, etc. Whatever the likes and interests of that person should be included in the plan.
