Hello Karen, Hope you are well. I just wanted to give you some feedback after visiting yo= ur Mom this morning.=20 Physically, I found that her gait has changed especially as she walks and tu= rns corners, she almost fell going to her bedroom from the living room. She a= lso does not lift her feet as much as she used to. I am concerned about her g= oing to the Cavendish Mall and mobilizing on her own, and crossing the stree= t. In terms of her medications, we all know that she does not always take her m= edications and at the times that she is supposed to take them. This morning I= checked her dosette, and the morning medications had not been taken. I don'= t even think she had eaten breakfast. I asked her if she had eaten breakfast= and taken her medication, she didn't remember if she had eaten breakfast an= d she insisted that she had taken her medication. I showed her the blister p= ack with her morning medications still there, but she insisted that she had t= aken them and showed me the empty part of the blister packs and told me that= the medications were in there. She refused to take the morning medications a= fter I told her she hadnt taken them, but I could see that she was having a d= ifficult time reading, figuring out, and processing what blister pack she ha= d to take and when. I am also concerned about her nutrition.=20 I'm not sure that she is eating when the caregivers are not there, most of t= he time her fridge is empty. I have asked the caregivers to make sure she al= ways eats breakfast, then they go to Cavendish Mall and I've asked them to m= ake sure that she eats lunch before they leave her. As well, I think it is taking her longer to accomplish her tasks, her bathin= g which she likes to do independently. I have asked the caregivers to check o= n her while she's in the bathroom, and a lot of times she is trying to decid= e, and get ready to take a bath, and it could take up to half an hour before= she actually goes in to take one.=20 I think her memory loss, repetition and disorientation have increased. She d= oes not remember that the caregivers were ever there, or when you last visit= ed. This morning she told me that she has never had a caregiver or a houseke= eper there before.=20 We have reached a point where I am quite concerned about her well-being and f= unctioning when she is alone. She is definitely a big fall risk.=20 I am very concerned about her judgement and her foresight. I also feel that s= he is neglecting herself when the caregivers or you are not there.=20 I will be calling you later to discuss.=20 Thank you, Maria Maria Licoudis, Registered Nurse, C.N.C.C, N.M.C Care Manager Home Care Assistance mlicoudis@homecareassistance.com www.homecareassitancemontreal.ca Office: 514-907-5065=
