Thank you for the update.

As per the agreement, invoices will be paid within 30 days of receipt. The invoice for April was not received until the 15th of May. If you do not receive payment after 30 days please let me know and I will follow up with the accounts department.

Thank you for faxing the May invoice to Jo yesterday.  Upon review, we noted that the hourly rate on the summary invoice (see attached) is incorrect.  The hourly rate on the individual invoices appears to be correct.  Please update and re-fax this as soon as possible as our Finance department is awaiting this.

As a reminder – please complete the monthly Key Performance Indicator (KPI) online form<https://forms.regionofwaterloo.ca/External/Community-Services/Senior-Services/HNSA-Service-Provider-Monthly-Submission-Form> for May.  Jo sent out a monthly reoccurring calendar reminder a few weeks ago with the link.  This online form, along with the invoice, is due no later than the 7th of the month following service provision (i.e. June 7th).
