As we discussed, I am a care manager with Home Care Assistance however the goal was to introduce myself to your mother as an independent social worker as an effort to develop a positive relationship where she will feel comfortable and be explore how she has been doing. I will be working closely with Timothy and it will be important for us to all ensure that we are in communication. Through my visits with your mother, I will be sure to update you as to how she has been doing, any changes that have been noticed and begin to address the need to increase care. As we have agreed, it will be important for your mother to be assessed medically. Following a doctor's assessment, we can ensure that our interventions are in line with the doctors recommendations. I am hopeful that I can initiate the conversation regarding the need to increase care and for your mother to develop a greater understanding of her needs. However it will be important that you and your brother speak to your mother regarding the need to increase care. As the goal is to ensure that your mother remains in her home for as long as possible, it is important for all of us to continue to reiterate that point however it is important to ensure her safety.
