Hello Brian, Dennis and Tim,

I received an email from Vashon stating that the above listed clients are
missing PAD'S and Consent Agreements and that he sent an email and received
no reply.

I can confirm that these required documents are missing from each client's
clear care files.

Can we please have these uploaded asap in order to ensure payments are made
in a timely manner.

It is my understanding that these are to be uploaded to clear care prior to
start of care.

Please advise on who we should contact in the future for these requests
moving forward.
