Hi Tim,

To grant Irin and myself access to Orofresh's Employer Health Tax account,
I need you to complete the attached form. Carolina previously initiated my
online access, but the setup was not finalized. The agency requires this
form to complete the process. I've also included Irin's name on the form as
a backup, given her location in Canada.

I would greatly appreciate your prompt attention to this matter.

Dear BC Ministry,

I am writing to request assistance with accessing Orofresh Enterprise's EHT
tax account (EHT # 83637 0056). Although my online access indicates that I
have full permissions, neither I nor my office has received the ITB
Registration Letter needed to complete the online registration.

To avoid further late filing and deposit penalties with the Ministry of
Finance, could you please reissue the ITB Registration Letter via email? I
have attached proof of my authorization to access the account. I have
contacted web support more than four times and was incorrectly informed
that I am not an authorized contact, preventing them from assisting me.
