What form do you need filled out each month? We do not receive the same form as Alberta because again, this is private insurance not part of a program. The client is not a veteran so there is no VA involved in this case. I will sign this form and send it to Blue Cross but how does that change how TheKey bills? I need to know what you need in order to submit the bills to the insurance company.
