T= here is a payment assignment form that needs to be filled out by both Vij= ay and TheKey. I have attached the blank form and the one that Vijay has = filled out. TheKey is yet to sign the form. Additionally, TheKey is askin= g for a letter from Blue Cross regarding the coverage.
