Very well written and thorough. I would like to see you break down each domain and comment on her performance and engagement...note if you have observed any changes since beginning. Refer to the individual activity notes per domain to help expand= . Language Domain: Mrs. Gang has really enjoyed activities revolving around the language module as they are often just challenging enough to remain interesting...please expand on which activities and performance Memory Domain: She often does not want to take part in any activities in the memory module as they are far too difficult that she eventually deems as, "too boring." Her short term memory is quite weak and something I plan to work on more frequently, with more engaging activities. For example, if shown a list of words for 2 minutes and then asked to recall as many of those words as possible, Mrs. Gang will argue that there are too many words, or that she is too tired or simply shat she doesn't know or cannot remember any. Mrs. Gang often gives up quite easily when a task is more difficult than expected....any other memory activities she has completed? Executive Functioning: Here I would discuss her judgement skills. How well has she done. Take a look at the activity notes for Executive Functioning....i see she likes mazes: Mrs. Gang really enjoyed this activity and asked to do it more often! I started with level two, some proved to be too simple, while others a lot of guidance was needed. Mrs. Gang seemed to have a very hard time focussing throughout the activity....see if there are any other relevant activities and add context. Visual Spacial Recognition: You completed a few activities such as Time Specifications: Did great with general times (1:30, 6:00 etc) had difficulty with specific times over 45 ( 7:53, 2:49, 12:58) could not differentiate between hour/minute hand when becoming anxious, also Visual Puzzels: Argumentative, did not think the cut up images built the completed ones. Performed well when guided....try and expand on these concepts. Attention: It seems you've only done one activity in this domain, which was recently Object Counting, your notes are: Mrs. Gang performed quite well but was very easily distracted by her cat Jesse. Had a more difficult time when asked to count abstract objects such as "things that make noise" (the counting was not at all an issue, it was determining which objects should be counted)...try and expand on her overall attention span though different activities. Then finish off with your summary....The CTM sessions with Mrs. Gang have been very successful. Since our first session until now, her progression in terms of engagement, cooperation, openness and optimism have been evident. Mrs. Gang is not shy and has absolutely no problem letting me know when she does
