It should be stated that we typically do not receive forms with an end date from MPI when they initiate a service request (from my experience). Typically they will reassess this person as we go along, and if they determine sufficient recovery then an end date for service is provided. The assumption is that we provide service until there is a defined ending. I will ask the client directly if they have any info to this effect and update you no later then tomorrow.
