I am not sure if this is in the live in hours since we have had an increase. Clear Care/Wellsky data shows both revenue and billable hours up. *Action: Reached out to John Champagne for insight* Funnel In reviewing the funnel B2B were down by 3. These three additions last week were from a previously stalled account. Riverview Medical Center. Mike Werner who is our HCL has made some connections with the Case Management team. We continue to promote the focus for continued referrals. We have 2 new seasoned HCL=E2=80=99s who come to us with extensive Private = Care experience. Jay Heenan formerly worked at Privatus as well as Amedysis. Lucy G. comes to us from Home Instead. She will be taking over for our former HCL-Rachel McKinney. She is very familiar with the area and several of Rachel=E2=80=99s former accounts. Most notable decline is Leads to Opportunities: We are down WOW by 13. In looking at the funnel at Closed Leads. The decline of 13 were from B2C leads that did not convert. The main reasons were client not reachable, client not financially qualified, or client not looking for our service *Action: Meeting with Cristal/Chris to look for opportunities to convert more leads to opportunities. * Ax Scheduled We have 3 individuals looking for care in September, 2 in rehab, and 1 starting next week. Ax Completed 1 unsure: looking for minimal hours/less than 3 family deciding: clients are resistant but family identifies needs 3 future SOC (Sept) 2 currently in rehab 1 Stopped responding (will be closed out tomorrow if no response) 3 in process (for August/September). 1 Tier 1 Couple Care Case/Meet and greet. Meeting with son discuss scheduling meet and greet for early next week (case expected to start by end of Aug/early Sept when family returns to NJ. *Additional Action Items* Continued focus on retention including CSM follow up of all Tier 1 (1 week after care begins and at 30 days). All clients who discharge now receive a call 30 days post discharge to identify plans change, recent hospitalizations, etc. Connect with the Director of Nursing at the Chelsea at Fairlawn. An incident occurred with a former client who went with our former HCL-Beth. Client is imminent death but the family didn=E2=80=99t want to make another= change in spite of the issue. DON is on vacation but our HCL Sasha and I will be scheduling a meeting with DON and try for the SW/CM team to reinforce our commitment to service.
