Pilot Integration Partnership - Introducing Qualified Families Dear Kristina, Thank you again for taking the time to meet with us. We appreciated the opportunity to share the concept of a pilot integration partnership and hear your perspective. We believe this program could bring real value to both Sunrise and TheKey: - For Sunrise, it introduces financially qualified, high-need families currently receiving 24/7 support at home, supporting client retention an= d providing transparency and coordination for families. - For TheKey, it allows us to continue providing companionship and support during the transition, strengthening the connection with families while respecting Sunrise=E2=80=99s clinical care model. This integration partnership leverages the complementary strengths of both providers: TheKey supports families at home, while Sunrise delivers premium care in the residence. To make this possible, the pilot would involve: - TheKey: Providing 20 hours per week of companionship for the first month to support the family and resident during the transition - Sunrise: Delivering all clinical care and residence services - Sunrise: Applying a $4,000 credit for this transition, reflecting the value of receiving a qualified new resident We will inform families that this is a preferred partnership with Sunrise, designed to make their loved one=E2=80=99s move as smooth and supported as possible. Importantly, TheKey=E2=80=99s role is strictly Companionship, Socialization, and 1-on-1 Safety/Wandering Support if needed, we are not a clinical alternative, but an Integration Partner supporting retention and continuity for residents and families. If this pilot works well, it will allow us to raise the standard of senior transitions, demonstrating how hand-in-hand collaboration between a private home care provider and a senior residence can deliver better service and outcomes for clients and families, while still respecting each company=E2= =80=99s business needs and structure. As discussed, the next step would be to share this concept with Melanie and Luca, to explore whether this partnership could be implemented. We would be happy to provide additional details or answer any questions as they review the proposal and to organise a virtual meeting. In return, we hope to be Sunrise=E2=80=99s preferred partner for families s= eeking home-based support, helping provide continuity and top-quality service.
