Client Deactivations (Last Week) A total of 11 clients were deactivated last week. Summary below: - 1 client on hold (GUIDE program): Expected to resume privately post-holiday. - 4 clients returned to baseline: No longer in need of services. - 1 client now cared for by family: Family moved in to take over care. - 3 clients are outside our scope of service and transitioned to a facilit= y rather than hire a different agency - 1 client deceased - 1 client switched agencies: - Used us for same-day service to be released from hospital, had another agency lined up to start the following Monday that he said was cheaper. We did not realize this until he called to cancel after = the first day of service. - Negotiation attempt was unsuccessful, but we=E2=80=99ve set a follow-= up to potentially win him back. Opportunities & New Starts - 2 new SOCs this week (Some start dates moved to post-holiday, but September is trending positively.) - 4 closed won - not started will be September! - 1 assessment scheduled for tomorrow, positive outlook for a start next week - 3 assessments completed and pending PandaDoc signatures - We are still losing opportunities for needing Skilled Care and Looking for State Paid Services. Our Lost Contact reason has decreased slightly= . Growth Initiatives & Winbacks - 17 closed-lost opportunities were contacted - 62 win-back calls made: - 2 clients confirmed to return in September - 2 existing clients increased service hours following QA reviews - 2 clients currently on hold are scheduled to resume services in Septembe= r GM Sales Activity - PPA Mohegan Tribe: Scheduled first-ever in-person meeting with their team, which opens new doors for collaboration. - Referral Partner Engagement: - Successful Lunch & Learn with Glade View and connected them with the GUIDE program for further alignment - Project ZipLock - I had 11 accounts - now have 22 accounts, 4 of which are SAP accounts= . - CT Hospice GUIDE Program: - Added additional hours for authorization - 2 clients confirmed to remain on private pay Thank you, Amanda J.
