start care 12/30. He would like the following schedule: Tues, Thursday and
Saturday.
He has a doctor's appointment on 12/30 and needs the Caregiver (CG) from 9
a.m. to 1 p.m. He needs the Caregiver (CG) to transport him to the doctor's
appointment that day. He said he was expecting Sarah to stop by rehab but
she called instead. He would like for her to send him consents to the
following gmail: rdmac1956@gmail.com and give him a call as soon as
