This happens every year around this time so they can cut hours. It's not a
fun process but we will have no choice but to implement it. My brain is not
very IT-oriented, so my only solution was to put these logs physically into
each home. Caregivers fill them out every hour. Our caregivers are not
trained on this kind of reporting so the results will be inconsistent.
Ultimately, we will do this but it won't be easy.
Anyway if any of your brains think more efficiently than mine please let me
know as if its not easy for caregivers it will not get done

Hello Home Care Assistance/The Key



VCH is undergoing a review for all HS clients receiving exceptional  HS
hours (above 8 hours per day) in the community.   VCH require your support
to understand all client’s scheduled and unscheduled care needs so we can
provide the right support and plan their care and future transition plans
into LTC facility.



We are requesting your assistance in providing our team with documentation
around the client care that your CHWs are providing:

Ask:

-          See CHW client care observation log and directions

-          Implement documentation requirements of CHWs effective for all
VCH clients by *Wednesday April 8th.*

-          Please forward observation logs to : david.shao@vch.ca or
eden.umali@vch.ca

-          First observation log due: *Monday April 20, 2026*



We will be providing more information as to plans to maintain / sustain the
Client Care Observation Logs as a regular expectation moving forward ; our
priority is to complete this first review for all current / new clients
(more information to come).



Going forward:  any new clients receiving more than 8 hours per day, can
you please provide this document for your CHWs.



We want to thank your for your ongoing support and quality care for NS
community clients to ensure clients are able to remain in their home
safely.
