This provided the CGM's an opportunity to spend time with the Caregivers, reset expectations, and strengthen relationships - because of this success we plan to have CG open house quarterly RIM Intake Process Following the conversation last week from the NE team in regards to the warm transfer from RIM to CSM, we have implemented it across the entire SE region . We have not had any transfers as of yet. GM scheduled meetings Yesterday, I had a breakfast meeting with my PPA account. She gave a referral we are following pending DC from rehab I was able to give a referral to The Waterford this morning from Holly as a way to break in with the new social worker . I was also put in touch with their marketing director for another contact. Meeting them tomorrow afternoon. My original contact who referred exclusively left (trying to locate him) so I am starting a new relationship Jaime and I are meeting w/ GM, Julia with Livewell to restructure our biweekly collaboration call to include more participation within the teams and have a structured agenda - as we continue to collaborate together, Brie was able to give them a referral this week In addition, meeting with Julia and potential new GCM that Holly=E2=80=99s = referral source uses. She will refer to both Livewell & The Key if she onboards with Live well Meeting new ED at Courtyard Gardens Tuesday Made intro calls / emails. Will be Cold calling all my new accounts next week to drop off collateral. In addition, I also like to bring a small plant as a welcome intro. My personal network, attending a senior event in my neighborhood next week *Jessic, ASM schedule* Jessica , ASM first full week since the move - Meeting scheduled with new GCM Ellen Emerson and doing Happy Hour with her this weekend Scheduling intro to The Madyson Senior LIving and Addington She has new prospect with a mobile clinic -Kure Mobile Working on Sponsorship with Palm Beach County Estate Planning Council for 9/18 *Brie, HCL schedule* Today a co-networking with Livewell tonight- Happy Hour with for Alzheimer's Community Care First Horizon Advisors meeting tomorrow Lunch and learn with Managed Care 9/9 First Timeslips event at Wellington Bay 9/11 *Controllable deactivations-6* 5 moved to a higher level of care 3 -went into inpatient Hospice, 2 went into memory care . 1 -Did not renew contract , bill rate $19/hr we tried to renew at a higher rate , they were not willing to do so *We met our July start of care retention goal - 80%* *Projection this week $2
