Wk 3 Homework: - Due by Next Week=E2=80=99s Session on Aug 9th - Go into ClearCare/Wellsky or Salesforce and Choose: 1. 1 client (in an Elephant tier or Turtle tier without dementia) 2. 1 client (in an Elephant tier or Turtle tier with dementia or cognitive changes) - Solutions Managers: select a client that has started in the last 2-3 weeks - Success Managers: select a client for whom you have completed 1 reassessment or QAV in the past 2-3 weeks. - Paste links into the =E2=80=98mystery box' space of your workbook and e= mail them to shadi@thekey.com with the subject line Care Plan First Take. - Save the links for easy access next wednesday and/or download or print. - DO NOT CHANGE ANYTHING.
