Joan Hanna Residence Le Maxwell Room 233   Mrs. Hanna is alert and oriented. Very smart aware lady. Has a prolapsed vagina and rectum. Must sit on her special cushion when in chair. Be = gentle when performing toileting needs. Requires verbal stimulation and companionship. Client very aware of her needs and will tell you. Likes = to do puzzles and loves music. Hard of hearing. Must speak loud. Mrs. Hanna is hearing impaired. SHE LIKES TO BE REFERRED TO AS MRS. = Mainly for companionship. She is hearing impaired and is experiencing some = anxiety and is withdrawing from social situations. It is difficult for her = family to get in touch with her because she can't hear the phone. Great if the caregiver can help her communicate with the family. She was a school principal. She likes music, cross word puzzles.     The second client is from 6 p.m. to 10 p.m. this Saturday, March 7. Jacqueline Surette-Bourque Residence Le Maxwell Room 232   Mrs Bourque is 92 and recently fell & fractured her hip; had pins put = in. Since her surgery, her medicine cocktail has caused her to hallucinate. They have assured the family that this is only temporary. Charting notes have been left for caregivers. It is extremely important the charting be done. During the day shift, caregiver must help with hygiene, bathroom, physio assistance, make sure she eats and drinks. Caregiver is also to provide companionship, help her walk with walker (she is weak, can only = walk 10 feet).   This schedule would repeat every weekend.
