Hi All!

For ease of reference, I have copied and pasted the LTCi info from the
Workplace page.  You can find the Workplace page here

to your client-facing team.  If you need team members added to this page,
please let Jeannie know and she can add those individuals.
The Long-Term Care Insurance Start of Care (SOC) Team needs 2-3 forms
completed in order to ensure Carriers reimburse our Clients (or TheKey,
when there is an Assignment of Benefit Customers) timely.
During the initial assessment and/or during following visits with the
client, the local office staff should ensure the proper forms are filled
out, ideally in advance of the clients first service date, to ensure there
is no delay in claims submissions or reimbursements.
TheKey *LTCi Intake Form*

   - This is a required form (available in Pandadoc and in our forms
   library)
   - This provides the SOC team with the client DPOA Status, DOB, Last 4 of
   Social Security, Carrier and Policy Information information needed to send
   invoices to the Carrier. TheKey does not share this form with the Carrier.

*Third Party Authorization Form*

   - This is a required form for AOB clients. Exceptions to this
   requirement can be discussed on a case by case basis.
   - Also called a HIPAA form, this is documented approval from the Client
   allowing the Carrier to verify benefit eligibility, specifics of a Clients
   coverage, and to discuss Reimbursement Status (Aka Claims status) with
   TheKey.

*An Assignment of Benefit Form*

   - Is a required form, per all Carriers, in order to submit a request for
   an Assignment of Benefit whereby TheKey will invoice the Carrier for
   services directly based on the benefits provided to the customer in the
   policy they purchased.
   - In the event a Client has set up an AOB without support from TheKey,
   please reach out to LTCi@thekey.com for direction on next steps.

*Some carriers require a specific form, which can be electronically signed
or contain a "wet signature" whereas others will accept forms provided by
TheKey.*
To navigate to the appropriate forms review the below list to identify the
Customer's Insurance Carrier, and navigate within each folder to that
specific Carriers Form based on the State and Office Location according to
the Client's assigned location in ClearCare.
