Hello Chicago,

I met via Zoom with Adam and Melissa Weitzman for their father Jeffrey Waitzman in Deerfield. They had originally met with Skip a number of years ago.
They have been disappointed with multiple service failures through the years, by multiple agencies. Their greatest NEED is effective Care Management, dependable staffing, and high quality care including engagement, outings, and quality of life. Their past experiences involve frequent caregiver call offs that have resulted in the family needing to provide coverage, which is unacceptable. Both Adam and Melissa are at the end of their ropes. They are desperate to find an agency that will be 100% responsible for their part of Jeffrey's schedule. They currently have three private caregivers who they will keep. Eventually they would like one to become an employee of TheKey.
They are in need of three 12 hour shifts, two of them on the weekends.
Caregiver should
-be able to cook
-willing to plan a weekend menu w Jeffrey
- be a driver with a car
-ensure medications are being taken in a timely manner (we discussed the need for a reminder box to be set up). I suggest we provide two referrals for a GCM in the Deerfield area
-Should be proactive in regard to activities, should engage,.
-The family would like to ensure that the caregivers understand what appropriate dress code means.
Jeffrey is left side week due to a stroke , definitely needs encouragement to exercise and will benefit from a good personality. match.
Adam and Melissa are very experienced in meeting agencies and state that they have heard promises from every agency only to find they still end up holding the bag when it comes to caregiver call offs.

They would like to speak with a Care Manag to understand the quality of the individual they'll be working with. They are looking for responsiveness and involvement.
Eventually, They will want to meet the care candidate.
Residential Hospice is in place in a long term format.

Can we have three time slots for today and/or tomorrow?
