Hi All! I made this quick reference and sent it to everyone on the Care Commitment open office hours! I wanted to also share with you so that you can cascade to others if needed. If the stated intention is greater than 30 hours/week *OR *greater than 30 days, then care commitment is optional. (But why not?! You get $500 towards care after you use the 5k!!) 1. Opt in and make advanced payment of $5k and receive $500 credit (Care Commitment CCA) 2. Opt out and do not receive $500 credit. (Universal CCA) If the stated intention is less than 30 hours/week *AND* less than 30 days, then the client has 2 options. 1. Advance Payment of $5k with a $500 credit (Care Commitment CCA) 2. Pay As You Go- commits to $5k in services over time. If they end services early they will be responsible for the remaining balance. Do not get $500 credit.
