Hi Nicole,

Great to see you in person for the first time this week. We had 3 good productive (drop in) calls together. Here is the field coaching form for your review.

Few top-line observations from our workwith:
Do not wait for appointments to get the ball rolling with your SAP accounts.
As demonstrated, drops ins done in a professional manner can help generate value for both the referral partner and us
More face-to-face meetings will generate more leads. More leads will generate more opportunities and more SOC!
You have done an incredible job in filling up the pre-call planning templates by key accounts
Plan your day in advance and include them in your google calendar, even if it is a drop in.
Your networking event was a huge success; in attendance were two geriatric care management organizations, two premium SLCs’ representative, end of life planner and home health owner
Engage Kathy from Proactive Seniors to conduct joint seminar at the Edwards

July Goals:
SOC- 3 (stretch target 4)
Actual- 0 (3 leads)
Projection: 1 contract from Shalem for staffing, companionship and nursing services. Could generate 1 or 2 SoC by end of this month. Please prioritize.

Revenue Goal: $10,000
MTD: $1,602
Forecast: $3,927

Pipeline:
Concerned about lack of SOC. We are mid-way into the month and need to be more diligent (right target with right messaging with right frequency of visits).

Key successes
Had productive meetings with 3 key SLCs
Potentially two contracts to be signed with Shalem and Union SLCs.
Successful start of partnership with one of the two key GCMs, Judy from Proactive Seniors
SLC partnership program targets identified

Action items:
Minimum requirement for care (8+ hours/week) is favourable to you; be strategic in how you want to position to bring on board new SLCs. You need to work with your operations team to send elite caregivers and watch for consistency of care. Please quote the higher end of the range for shorter cases ($39-$40). Our objective is to get these shorter hour cases to be Elephants and Turtles, eventually.

1) Senior Living Community (SLC) partnership project - keep me posted on the status of this project.
2) Bring a Senior Advisor/GCM to a SLC - Identified Stephanie Chan as the senior advisor. Update us me on our next 1-to-1 meeting on Tuesday.
3) Weekly report - to be completed by end of the week. Email it to the sales leadership team and Chuck.
4) Coordinate meeting with Proactive Seniors (Kathy) and Tim Thomas (part of Growth Initiative/executive intervention program)

Management Intervention:
Chuck and I will ensure you are included in the office updates and create more of amicable environment.

Best Regards,

Matt
