@Cristal Munoz​ @Timothy Thomas​ We have never had a 12 hr a week min. We will take a client for 1 day at 4 hours. I have been looking at the zip codes and I have to manually look up each one. I have sent several spreadsheets with zip codes and they are still not correct.

Chatter Notes: Jerry’s daughter Claire reached out to inquire about care for her dad Jerry. Jerry worked with TheKey in 2022 during his transition to The Courtyard Of Sussex due to his dementia. Jerry had a fall on Friday and was taken to the hospital and released that same day. When he returned home he had two more falls. He went from using a walker to using a wheelchair. He is currently at Community Memorial Hospital in the ER (W180N8085 Town Hall Road, Menomonee Falls, WI 53051). Claire is waiting for Jerry to be transferred to a room. There is no active discharge date due to Jerry’s O2 levels being low and him being on oxygen. Claire would like to have someone come to the hospital for an assessment so that she can have things in order before Jerry’s discharge. Claire is looking for overnight care for Jerry for safety concerns. She did mention daily hours from 5pm - 8am. Claire would like to meet with the care team on Tuesday, December 31, 2024 in the afternoon. She will update the staff on the room number that Jerry is in once she receives this information.
